Forman; daughters Denise Lawhorn and Diane Forman; grandson Neil Lawhorn; grandpuppies Loki, Dizzy, and Maui brother Daniel (Dollie) Adam; sister-in-law Catherine McLaughlin; nieces and nephews. Visitation Wednesday, March 15th, after 4:30 p.m. with Vigil Service 6:30 p.m. Bethany Funeral Home Chapel. Mass of Christian Burial Thursday, March 16th, 10:30 a.m. at St. Bernadette Catholic Church (7600 S. 42nd St.). Interment Omaha National Cemetery.
